First Installment of $500 Due on Tuesday, January 12,
2010.
Full Payment deadline: Friday, February 5, 2010.
- All checks must be made to the
Iowa Soccer Club.
- Please write "ISC Men's League Session II" and
your team name in the memo section of your check.
- The $500 first installment must
be made by Tuesday, January 12, 2010.
- Your team representative must send all checks
together. It is preferred that you send the ISC one check for all team
members. However, we will accept separate checks for individual
players. We cannot accept cash.
- All checks for the first installment must be cleared
before your team's first
game. If any checks do not clear, your team will not be allowed
to play in the first game of the league.
- Full payment must be made by Friday, February 5,
2010. If payment is not made on that night of league play, your
team will not be allowed to play.
How to register
There are two steps that your team must follow to register for the
league this year.
Step #1-Each Player Must
Fill out the Player Consent Form Online
The player consent form must be filled out online.
Therefore, all players who would like to play in the league must
go online and complete the form by choosing one of the two links below
to register. There is no charge to fill out the player consent
form unless the form is filled out after the deadlines listed below.
Team payment will be handled in step #2. All adult
players must fill out the
player consent form by 11:59 PM on Thursday, January 14, 2010.
All high school aged
players must fill out the player consent form by Monday, January 11,
2010 at 11:59 PM. If high school aged players fill out the
registration form after January 11, they will be assessed a $15.00 fee
in order to register on Tuesday, January 12. However, if the high
school players do not fill out the waiver by 5:00 PM on Tuesday,
January 12, they will not be included at all in the league.
Make sure that you fill out the appropriate form. Adult

Step #2- Team Registration
and Payment

your team. You can make changes to your roster through the online
system up until 11:59 PM Thursday, January 14, 2010.
- Send in a check for either the first installment of
$500 or the full $1,000 payable to ISC to the ISC office at 220
Lafayette Street,
Iowa City, IA, 52246. If you pay only the first installment at
this time, the second installment of $500 must be received by Friday,
February 5, 2010.
